I was right:
If you email a PDF file to Kindle and place the word "convert" in the
subject field of the e-mail it will be converted to a format that allows you
to use the "Aa" button on Kindle to change font sizes.
This implies that the file is converted using the text layer of the pdf. The
reason it doesn't do this automatically is that the pdf may contain
pictures, diagrams etc. The convert in the subject line tells the convertor
to ignore such things, i.e. look at the text rather than the graphical layer
within the pdf.

It is certainly the case that the Kindle reader cannot use the TTS on an
imported PDF however it gets there.  This probably because the PDF viewer
does not analyse the text layer of the PDF which is what is required for the
TTS to be able to read.  Conversion to TXT format will therefore be the only
way.

I think you need to convert the pdf to txt to get kindle to read it.

There's something about pdf not being accepted by the email conversion tool
